You'll need the following:
- Copy of Team Cover Sheet
- Copy of Team Roster
- Copy of each player's current report card
- Team Photo

OPTION #1:  Upload to Dropbox
You can upload a pdf with all of the documents in it or a compressed zip file with the documents labeled appropriately.

    Players must have the following to be certified:

    • Proof of Age: Copy of birth certificate or Passport
    • Proof of Grade: Copy of most recent report card
    • Proof of Address: Copy of utility bill
    • Waiver(s) if applicable

    Step 2) Online Certification |  All teams must add their team online for National certification
